~~~ Spay/Neuter Agreement ~~~

(Not applicable if cat/kitten is purchases as a breeder)

 1.      Purchaser agrees to alter this cat/kitten within the suggested time frame of 6-8 months.:

 2.      Purchaser will provide Seller with proof of altering in the form of a dated receipt from the certified veterinarian that includes a description of the animal involved. When the Seller receives such form, Seller will provide papers for registration.

 3.      If purchaser fails to send proof of procedure by the  end of the time frame listed above, registration papers will be forfeited.

 4.      Should there be a medical reason as to why this cat/kitten should not be altered, the Purchaser should contact the seller with a written statement from a veterinarian stating the reason. The Seller will not be responsible for any veterinarian expenses.

I accept this altering agreement, and agree, to abide by the terms thereof:
